29 CFR 1903.19(d)(1): The employer must submit to the Agency, along with the information on abatement certification required by paragraph (c)(3) of this section, documents demonstrating that abatement is complete for each willful or repeat violation and for any serious violation for which the Agency indicates in the citation that such abatement documentation is required.  The employer did not provide documentation regarding the correction of the foolwoing cited hazards:  a.  Employees were required to work on apltaform, which was not designed, constructed, or loaded by a qualified person.  b.  Employees were working in a scaffold platform, which wasnot attached to the forks of the lift.  c.  employees working in a scaffold platform were not protected from falls to lower levels.  d.  Employees installing tar paper on a newly constructed residence were not protected from falls to lower levels.  e.  Employees installing tar paper on a newly constructed residence were not provided fall protection training prior to working on the roof.

29 CFR 1926.451(a)(6): Scaffolds were not designed by a qualified person and were not constructed and loaded in accordance with that design. Non-mandatory Appendix A to this subpart contains examples of criteria that will enable an employer to comply with paragraph (a) of this section.                        Employees were working on a platform, of which, the platform was not designed, constructed, or loaded by a qualified person in accordance with that design.

29 CFR 1926.451(c)(2)(v): Fork-lifts should not have been used to support scaffold platforms unless the entire platform is attached to the fork and the fork-lift is not moved horizontally while the platform is occupied.                          Employees were working in a scaffold platform, of which, the entire paltform was not attached to the forks.

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level. Paragraphs (g)(1)(i) through (vii) of this section establish the types of fall protection to be provided to the employees on each type of scaffold. Paragraph (g)(2) of this section addresses fall protection for scaffold erectors and dismantlers.  Note to paragraph (g)(1): The fall protection requirements for employees installing suspension scaffold support systems on floors, roofs, and other elevated surfaces are set forth in subpart M of this part.                  Employees working on a scaffold platform were not protected from falling to lower levels.

29 CFR 1926.501(b)(13): "Residential construction." Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure. Exception: When the employer can demonstrate that it is infeasible or creates a greater hazard to use these systems, the employer shall develop and implement a fall protection plan which meets the requirements of paragraph (k) of 1926.502.  Note: There is a presumption that it is feasible and will not create a greater hazard to implement at least one of the above-listed fall protection systems. Accordingly, the employer has the burden of establishing that it is appropriate to implement a fall protection plan which complies with 1926.502(k) for a particular workplace situation, in lieu of implementing any of those systems.            Employees installing tar paper on a newly constructed residential home, were not protected from falling to lower levels.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee who might be exposed to fall hazards. The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards.                        Employees installing tar paper on a residential roof were not provided with fall protection training prior to working on the roof.
