29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use:  (a) On or about April 3, 2013, and at times prior thereto, employees were exposed to eye damage while mixing or moving corrosive products such as, but not limited to, Lime with a pH value of 12.2 at the mud mixing area of Ensign Drilling Rig 46 located at L Hoiby 159-94-198-30-2H near Battleview, ND.

29 CFR 1910.303(b)(7)(iv): There were damaged parts that adversely affected safe operation or mechanical strength of the equipment, such as parts that were broken, bent, cut, or deteriorated by corrosion, chemical action, or overheating:  (a) On or about April 3, 2013, and at times prior thereto, employees were exposed to electrical shock hazards while plugging equipment into an electrical outlet with a faceplate that was not securely fastened to the outlet of the #2 Pump House of Ensign Drilling Rig 46 located at L Hoiby 159-94-198-30-2H near Battleview, ND.    (b) On or about April 3, 2013, and at times prior thereto, employees were exposed to electrical shock hazards from a live contact on the end of a flexible cord sticking out of a junction box with electrical tape to cover the end in the #2 SCR House of Ensign Drilling Rig 46 located at L Hoiby 159-94-198-30-2H near Battleview, ND.
