29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards: (a) On or about 04/11/2013 - at 6247 Amber Hills Road, Trussville, AL 35173, the employer's training program did not enable each employee to recognize the hazard of falling when using a step bolt, climbing peg, chicken peg or other open-ended item as anchorage where the rebar hook or lanyard can roll off or become disengaged from the open end.
