29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:  For the maintenance employee who performed servicing or maintenance on equipment including but not limited to the laundry washers and dryers,  a lockout tag out program had not been implemented including but not limited to:      a) written lockout tag out program     b) the use of singular locks and devices     c) coordination with outside contractors     d) periodic inspections     e) lockout tag out training

29 CFR 1910.132(a): Personal protective equipment was not used and/or maintained in a sanitary and reliable condition when necessary whenever hazards capable of causing injury and impairment were encountered:  Employees working at Big River Nursing & Rehab, LLC, located at 6400 The Cedars Court, Cedar Hill, MO 63016, used defective and damaged personal protective equipment.  Specifically, eye protection was not available in the laundry for employees who changed out corrosive chemicals.  Employees working in the soiled utility room and kitchen used damaged goggles while rinsing soiled linen and cleaning ovens; the goggles were severely scratched and scuffed, which impacted the employees ability to see through them.
