29 CFR 1910.305(b)(1)(i): Conductors entering cutout boxes, cabinets, or fittings shall be protected from abrasion, and openings through which conductors enter shall be effectively closed. On or about April, 4, 2013, in the meat smoker room, a 220 volt cable was observed with approximately 2 inches of the outer protective covering missing outside of the junction box.

29 CFR 1910.305(b)(1)(iii): Where cable is used, each cable shall be secured to the cabinet, cutout box, or meter socket enclosure. However, where a cable with an entirely nonmetallic sheath enters the top of a surface-mounted enclosure through one or more nonflexible raceways not less than 457 mm (18 in.) or more than 3.05 m (10.0 ft) in length, the cable need not be secured to the cabinet, box, or enclosure provided all of the following conditions are met: On or about April 4, 2013, in the meat smoker room, a 220 volt cable was observed with approximately 2 inches of the outer protective covering missing outside of the junction cabinet. The cable was not properly secured to the box nor attached by a raceway.

29 CFR 1910.1200(e)(1): Employers shall develop, implement, and maintain at each workplace, a written hazard communication program which at least describes how the criteria specified in paragraphs (f), (g), and (h) of this section for labels and other forms of warning, safety data sheets, and employee information and training will be met. On or about April 4, 2013, at the Bellevue Meat Processing facility, the employer failed to develop and implement a written hazard communication program that reflected the use of a chemical cleaner and mineral oil at the establishment.
