29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system:    a) Eric Widney dba ESW Construction, Perrysburg, Ohio:  On or about April 9, 2013, at address 15859 Corner Brook Ct., Perrysburg, Ohio, the employer did not ensure that employees were protected from falls while installing new roofing materials to a steep sloped roof (5:12) on a residential type structure.  An employee was exposed to a potential fall of approximately 11.3 feet to the ground below.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee who was exposed to fall hazards that enabled each employee to recognize the hazards of falling and train each employee in the procedures to be followed in order to minimize these hazards:  a) Eric Widney dba ESW Construction, Perrysburg, Ohio:  On or about April 9, 2013, at address 15859 Corner Brook Ct., Perrysburg, Ohio, the employer did not ensure that employees had been trained in the appropriate use of fall protection, such as but not limited to the use of personal fall arrest systems.  An employee was exposed to falls of approximately 11.3 feet to the ground below, which could result in permanent disability or death.
