29 CFR 1926.20(b)(4): The employer did not ensure that the employee(s) required to operate equipment and machinery was qualified by training. (a) On or about April 9, 2013 at 5440 Prospect Kansas City Missouri 64127. Employees were exposed to a struck by hazard when it was discovered during inspection that the employer had not established a training program for employees who are required to operate a Sky Track fork truck.
