29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents. On or about April 12, 2013, at the jobsite employees were observed not wearing eye protection while using hammers and pneumatic nail guns to install roofing materials. The employees were exposed to eye injury hazards.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: On or about April 12, 2013, at the jobsite employees were observed installing roofing materials on a roof more than 6 feet high without wearing personal fall arrest systems or having any other type of fall protections provided. Employees were exposed to fall hazards.

29 CFR 1926.1053(b)(1): Because of the portable ladder's length, the ladder side rails did not extend to at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access and the ladder was not secured at its top: On or about April 12, 2013, at the jobsite employees were observed using portable ladders to access the roof that the side rails did not extend at least 3 feet (.9 m) above the upper landing surface and were not tied off at the top. Employees were exposed to fall hazards.
