29 CFR 1926.95(a):   Personal protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, was not provided, used, or maintained in a sanitary and reliable condition it was necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation, or physical contact.  At the project site on April 10, 2013, eye protection was not worn while an employee applied "Sheetrock All Purpose Joint Compound" onto the ceiling. The material safety data sheet stated to wear the appropriate eye and skin protection.

29 CFR 1926.405(g)(2)(iv):   Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws:  At the front of the project site on April 10, 2013, the outer insulation of a 50 foot extension cord, was not secured to the plug.  The cord was used to supply 110 VAC power to a RIDGID 4-Amp Pro Compact Reciprocating Saw. Employee used the Reciprocating Saw to cut sheetrock.

29 CFR 1926.451(e)(1):   When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, sturcure, personnel hoist, or similar surface was not used.    At the project site on April 10, 2013, a portable ladder or other acceptable means of access was not provided to access the mobile scaffold.  The working platform was about three feet from the floor.
