Curren Advanced Wastewater Treatment Plant - for employees conducting a horizontal confined space entry into an 8 ft diameter sewer pipe through the 90 degree bend opening leading to meter vault # 2 - building 7, on or about 4/11/13. Employees entered the confined space without the use of continuous forced air ventilation. The venturi pneumatic air mover was set up at the entrance of the sewer pipe to exhaust air out. The other end of the pipe still had the meter valve in place and was closed, so no forced air ventilation was provided. The pipe was approximately 193 feet long.    b.  Howard F. Curren Advanced Wastewater Treatment Plant - for employees conducting a horizontal confined space entry into an 8 ft diameter sewer pipe using the ladder near sedimentation tanks #5-8, on or about 4/10/13. Employees entered the confined space without the use of continuous forced air ventilation into the sewer pipe. The venturi pneumatic air mover was set up at the top of the ladder and forced air down in front of the opening but did not force air into the horizontal pipe where the employee walked approximately 5- 20 ft.

Employees entered the confined space without the atmospheric test results documented on the entry permit. This section of pipe was not provided with forced air ventilation.    b. Howard F. Curren Advanced Wastewater Treatment Plant - for employees conducting a horizontal confined space entry into an 8 ft diameter sewer pipe through the 90 degree bend opening leading to sedimentation tanks #5-8, on or about 4/11/13. Employees worked in the permit required confined space for approximately 4 hours. Periodic atmospheric conditions were not recorded. Exit atmospheric conditions were not recorded on the entry permit.   c. Howard F. Curren Advanced Wastewater Treatment Plant - for employees conducting a vertical confined space entry at Junction Chamber #2, on or about 4/9/13. Employees re-entered the confined space at 10:30 am without documentation of atmospheric test results on the entry permit. The employee exited at 11:05 am without exit readings documented on the entry permit. The entry permit lists the last exit reading at 9:02 am which is before the re-entry occurred.   d.  Howard F. Curren Advanced Wastewater Treatment Plant - for atmospheric readings at the opening to the pipe section leading to sedimentation tanks # 5-8, on or about 4/9/13. A 4-gas meter was lowered into the ladder opening and was used to measure the atmospheric conditions at approximately 12:05 pm at opening. The hydrogen sulfide levels were high and caused the gas meter alarm to activate.  When the alarm activated the entry permit was voided; however, the atmospheric levels were not recorded on the entry permit to inform employees of the dangerous levels.

Howard F. Curren Advanced Wastewater Treatment Plant - for employees conducting a horizontal confined space entry into an 8 ft diameter sewer pipe through the 90 degree bend opening leading to meter vault #2 - building #7, on or about 4/11/13. Employees were exposed to hydrogen sulfide gas, in that, a gas detector was not used to determine the internal atmosphere at the location where employees were conducting cleaning activities. Employees were working approximately 125 ft inside the sewer pipe. One employee died and another employee was hospitalized due to hydrogen sulfide overexposure. Forced air ventilation was not provided. In addition, two other employees were working up to 20 ft inside the sewer pipe without using a gas detector.     b. Howard F. Curren Advanced Wastewater Treatment Plant - for employees conducting a horizontal confined space entry into an 8 ft diameter sewer pipe through the 90 degree bend opening leading to sedimentation tanks #5-8, on or about 4/11/13. Employees were exposed to hydrogen sulfide gas, in that, a gas detector was not used to determine the internal atmosphere at the location where employees were conducting cleaning activities. Employees were working up to approximately 140 feet inside the sewer pipe. The 90 degree bend was open to the outside atmosphere. A venturi pneumatic air mover (bazooka) was placed at the entrance to the pipe to blow (supply) air. The other end of the pipe was open to the atmosphere (sedimentation tanks). Another pneumatic air mover was placed at the ladder to exhaust air out of the space. The bazooka was placed near the top of the 10-12 ft ladder. A Gas Alert Max XT meter (serial #MA211-037024) was placed at the entrance of the 90 degree bend; however, employees did not have a gas detector at the location of work inside the sewer pipe.     c. Howard F. Curren Advanced Wastewater Treatment Plant - for employees conducting a horizontal confined space entry into an 8 ft diameter sewer pipe through the 90 degree bend opening leading to sedimentation tanks #5-8, on or about 4/10/13. Employees were exposed to hydrogen sulfide gas, in that, a gas detector was not used to determine the internal atmosphere at the location where an employee carried the vacuum hose approximately 5- 20 ft into the sewer pipe. The 90 degree bend was closed to the outside atmosphere. A Gas Alert Max XT meter (serial #MA211-037024) was lowered down the vertical ladder and was hung at approximately 5' - 5'-5" high from the bottom of the pipe.; however, employees did not have a gas detector at the location of work.

Two employees entered the confined space without wearing a harness and retrieval line.       b. Howard F. Curren Advanced Wastewater Treatment Plant - for employees conducting a horizontal confined space entry into an 8 ft diameter sewer pipe through the 90 degree bend opening leading to meter vault #2 - building #7, on or about 4/11/13. Three employees entered the confined space without wearing a harness and retrieval line. Two employees became unconscious due to overexposure to hydrogen sulfide gas at approximately 125 ft inside the sewer pipe. No retrieval system was in use to remove employees from the hazardous environment. One employee died. Six rescuers also went inside the confined space without a harness and retrieval line.
