They were not using ground fault circuit interrupters.

Paragraphs (g)(1)(i) through (vii) of this section establish the types of fall protection to be provided to the employees on each type of scaffold. When the pump jack has to pass bracing already installed, an additional brace shall be installed approximately 4 feet (1.2 m) above the brace to be passed, and shall be left in place until the pump jack has been moved and the original brace reinstalled:    On or about 4/17/2013, 850 E. 220th Street    a) The pump jack scaffold did not have support brackets at the bottom. The scaffold was approximately 23' high.
