29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section. a) Oxford Mills, 100 West Oxford Street, Philadelphia, PA, Courtyard Area- Employees were exposed to fall hazards from a scaffold erected to a height above the lower level of approximately 35 feet, while making repairs to the existing brick building, due to the lack of midrails in several locations.

29 CFR 1926.451(g)(4)(ix): Each midrail was not capable of withstanding, without failure, a force applied in any downward or horizontal direction at any point along its top edge of at least 75 pounds (333 n). a) Oxford Mills, 100 West Oxford Street, Philadelphia, PA, Courtyard Area- Employees were exposed to fall hazards from a scaffold erected to a height above a lower level of approximately 35 feet, while making repairs to the existing brick building, in that the steel rail sections utilized as midrail in many areas would swing out with a very small application of force.
