29 CFR 1926.403(i)(2)(i): Live parts of electric equipment operating at 50 volts or more were not guarded against accidental contact by cabinets or other forms of enclosures, nor by any of the means listed in subparagraphs (A) through (D) of this paragraph.  Location: 68 West 119th Street, New York, NY 10026, Cellar Location North Wall.  On or about April 26, 2013;  a). An employee was working in the cellar area in close proximity of the live 240 volt electrical service panels which were not guarded as he ran plumbing pipes for the potable water supply for the residence.

29 CFR 1926.501(b)(4)(i): Each employee on a walking/working surface was not protected from falling through holes (including skylights) more than 6 feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around said hole (Stairway to floor below).  Location 68 West 119th Street, New York, NY 10026, Basement, Cellar and First Floor Levels   On or about April 26, 2013;  a). Employee was exposed to a 8 foot by 8 foot flor hole opening where the stairway to access the cellar and no form of fall protectoin wsa provided or used.

On April 26, 2013 and other times prior thereto;   a). Employees were not trained to recognize and minimze fall protection.  An employye was exposed to  floor opening and was not trained.

Location 68 West 119th Street, New York, NY 10026, cellar, basement and First Floor Levels.   On or about April 26, 2013;   a). Employee esrd the stairway to get to the basement and it was not equipped with handrails.
