29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.  Cross-braces were used as a means of access:     (a) Jobsite, Ellsworth & Summerlea, Pittsburgh, PA:  On or about 05/06/13 employees were observed climbing the scaffold system between the 4th and 6th levels.  No access/egress was provided beyond the 4th level for employees who were working on the upper levels.

29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii):     (a) Jobsite, Ellsworth & Summerlea, Pittsburgh, PA:  On or about 05/06/13 employees were working on the 5th and 6th levels of the scaffold that were not fully decked.  Approximately 18 inches between the uprights on each of these levels was not decked.       The employer, Lane Plastering, Stucco & Painting was previously cited for a violation of this Occupational Safety and Health Administration standard or its equivalent standard 29 CFR 1926.451(b)(1), which was contained in OSHA Inspection Number 311326250, Citation Number 2, Item Number 1, issued on June 11, 2008 with respect to a workplace located at West Mifflin, PA.

29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms:      (a) Jobsite, Ellsworth & Summerlea, Pittsburgh, PA:  On or about 05/06/13 employees were working from a scaffold that did not have guardrail systems installed along all open sides and ends.  The scaffold lacked midrails at the front and ends of the system.  Employees working on the 5th level of the scaffold were exposed to fall hazards of approximately 32 feet.     The employer, Lane Plastering, Stucco & Painting was previously cited for a violation of this Occupational Safety and Health Administration standard or its equivalent standard 29 CFR 1926.451(g)(4)(i), which was contained in OSHA Inspection Number 311326250, Citation Number 1, Item Number 1 and was issued on June 11, 2008, with respect to a workplace located at West Mifflin, PA.

29 CFR 1926.454(c)(2): Employees were not retrained where changes in the types of scaffolds, fall protection, falling object protection, or other equipment present a hazard:     (a) Jobsite, Ellsworth & Summerlea, Pittsburgh, PA:  On or about 05/06/13 employees required scaffold safety and fall protection retraining.  Employees were working from a scaffold that did not have adequate fall protection systems (guardrails) in place and had decking deficiencies.     The employer, Lane Plastering, Stucco & Painting was previously cited for a violation of this Occupational Safety and Health Administration standard or its equivalent standard 29 CFR 1926.454(c)(2), which was contained in OSHA Inspection Number 311326250, Citation Number 1, Item Number 3 and was issued on June 11, 2008, with respect to a workplace located at West Mifflin, PA.
