29 CFR 1910.215(b)(9):  The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch:    a) Shop area, 2131 North Charles Street, Pittsburgh, PA 15214:  Observed on or about March 21, 2013, the gap between the tongue guard and the abrasive wheel of a bench grinder was approximately 2 inches (left wheel) and 1.5 inches (right wheel).  Employees using the United States Electrical Tool Co. bench grinder (model number 10UT, serial number 321325) were exposed to struck-by and fire hazards due to the improperly adjusted tongue guards.
