Adequate personal protective equipment (chemical splash goggles, face shields or other types of eye and face protection) was not required to be used by employees. The employer did not maintain the documentation for the hazard assessment conducted in the workplace. Employees were exposed to chemical exposure hazards to the eyes and face, on or about May 10, 2013.  b) Garage Area, 525 East Main Street, Plains, PA: Employees drained and filled radiators containing antifreeze (liquid chemical). Adequate personal protective equipment (chemical splash goggles, face shields or other types of eye and face protection) was not required to be used by employees.
