29 CFR 1904.7(b)(3): When an injury or illness involves one or more days away from work, the employer did not record the injury or illness on the OSHA 300 Log with a check mark in the space for cases involving days away and an entry of the number of calendar days away from work in the number of days column:   This violation was discovered on 5/23/2013 at a job site located at 185 Dulce De Maria St. Hagatna, GU 96910 where the OSHA Form 300 for calendar year 2012 incorrectly identified a check mark under "Other recordable cases" instead of placing the check mark under "days away from work."

29 CFR 1904.32(a)(1):  the employer did not review the OSHA 300 Log to verify that the entries are complete and accurate, and correct any deficiencies identified:  This violation was discovered on 5/23/2013 at a job site located at 185 Dulce De Maria St. Hagatna, GU 96910 where the OSHA Form 300A for calendar year 2012 indicated "one" for total number of days away from work when there were two such cases.
