29 CFR 1926.100(a): Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns.      On or about May 22, 2013, on the west side of the work site, employees were not wearing hardhats while working below other employees that were installing metal wall panels from an elevated work platform.  The employees working below were exposed to struck-by hazards from falling objects.

29 CFR 1926.404(f)(6): The path to ground from circuits, equipment, or enclosures was not permanent and continuous.      On or about May 22, 2013, on the west side of the work site, employees installing metal wall panels utilized 2 extension cords that were missing the ground pins exposing the employees to electrical hazards.

29 CFR 1926.451(c)(2)(iv): Front-end loaders and similar pieces of equipment were used to support scaffold platforms without being specifically designed by the manufacturer for such use:Note:  Preamble to Subpart L promulgated new paragraph (c)(2)(iv) & (v) in final rule to provide guidance. Added provision requires that in the case of fork-lifts, the entire scaffold platform be secured to the forklift.     On or about May 22, 2013, on the west side of the work site, employees installing metal wall panels worked from a platform that was elevated by a B-8400 Sky Trak that was not constructed in accordance with American National Standard Institue (ANSI) B56.6. exposing the employees to a fall hazard of approximately 24 feet.
