29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: On or about May 24, 2013, the employer did not ensure that the employees performing roofing activities on a medical complex roof were protected from fall hazards. Employees working on a 6 in 12 pitch roof were exposed to a fall hazard of approximately 15 feet to the ground below.

29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigied support that will not deflect, and/or a grasping device was not provided to assist emmployees in mounting and dismounting the ladder. On or about May 24, 2013, employees performing roofing activities were exposed to fall hazards when accessing the roof by the means of an extension ladder that was not 3 feet above the landing.
