At the jobsite located at 12509 Donahoo Rd. Kansas City, Kansas:  Employees engaged in roofing activities using pneumatic nailers were exposed to struck-by hazards in that eye protection was not provided.

At the jobsite located at 12509 Donahoo Rd. Kansas City, Kansas:  Employees engaged in residential roofing activities from a 5:12 pitch roof were exposed to fall hazards of approximately 9 feet from the eave of the residence to the ground below in that fall protection was not provided.
