Plasto-Tech Corporation worksite located at 708 Lowell St., Elyria, Ohio: On or about June 25, 2013, the employer did not ensure the Brown Thermo-Forming press was adequately guarded to protect the operator at the loading station. The employee was exposed to caught between and struck by hazards while operating the press.
