On or about April 26, 2013, and times prior thereto, the employer did not develop, document and implement a lockout/tagout program consisting of written procedures, training of authorized employees, training of affected employees, and periodic inspections of the program procedures.  Personnel performed servicing/maintenance on equipment that included but was not limited to laundry dryers.
