The depth of the floor hole was 21.5 inches.  B.)  Travaglini Enterprises, Inc. dba All American Gas & Car Wash, 18136 Conneaut Lake Road, Meadville, PA  16335, car wash area, on or about February 27, 2015 and times prior thereto:  A 10.75-inches by 4-inches gap between the concrete and metal sheeting creating a floor hole.  The depth of the floor hole was 9.75-inches.

Employees work with a variety of chemicals that could contact the eyes.  These chemicals included but are not limited to the Misco Vehicle and Transportation Wash Concentrate.  Employees dispense Misco Vehicle & Transportation Wash concentrate, and dilute the chemical with water for purposes including, but not limited to, washing the exterior of vehicles.  The Misco product is potentially damaging to the eyes in that it has a PH of at least 12.5.

Travaglini Enterprises, Inc. dba All American Gas & Car Wash, 18136 Conneaut Lake Road, Meadville, PA  16335, car wash area, on or about February 27, 2015 and times prior thereto:  Control wires from the car wash control box to the car wash were spliced with wire caps.   The splice was not contained in a box or conduit body. The wires were not utilized in a continuous length.      B.)  Travaglini Enterprises, Inc. dba All American Gas & Car Wash, 18136 Conneaut Lake Road, Meadville, PA  16335, car wash area, on or about February 27, 2015 and times prior thereto:  The cover for the junction box conduit was not seated and affixed to the conduit body.  The junction box conduit was located near the control box and housed wiring that was routed from a control box to the car wash.     C.) Travaglini Enterprises, Inc. dba All American Gas & Car Wash, 18136 Conneaut Lake Road, Meadville, PA  16335, car wash area, on or about February 27, 2015 and times prior thereto:  The condutors were routed through a small hole at the back of the junction box conduit.  The wires were not protected from abrasion as they passed through the hole.  The junction box conduit was located near the control box and housed wiring that was routed from a control box to the car wash.     D.)  Travaglini Enterprises, Inc. dba All American Gas & Car Wash, 18136 Conneaut Lake Road, Meadville, PA  16335, cashier's booth, on or about February 27, 2015 and times prior thereto:  The flexible metal conduit was pulling away from the fitting/connection point on the junction box.  The junction box powered the lights in the cashiers booth.
