29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:   A.)  Port Erie Plastics Inc.; Extruder Reel  - On or about 5/11/13, and at times prior thereto, one or more methods of machine guarding were not utilized to protect personnel performing activities associated with connecting the pipe to the reel (threading the reel).  The condition exposed personnel to unguarded moving parts and pinch points.  The pinch point was created between the moving reel/cross bars and the stationary A-Frame.  The conditions existed on the extruder lines including but not limited to Extruder Reel #702.
