29 CFR 1910.212(a)(1): Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices,electronic safety devices, etc.   a.  At the work site located at 22111 Jericho Road, Glenmont, Ohio, the employer had not provided adequate guarding on a machine used to convert commercial paper rolls into printed books, such as childrens coloring, activity and watercolor books and word search and crossword puzzle books.                1.  The Goss Subway printer, serial number unreadable, was not adequately guarded in that the open face of the employee pass-through area #1 had spinning moving parts at the point of operation.  There were no sensors or guards at the point of operation.              2. 29 CFR 1910.219(c)(4)(i): Projecting shaft ends shall present a smooth edge and end and shall not project more than one-half the diameter of the shaft unless guarded by nonrotating caps or safety sleeves.   a.  At the work site located at 22111 Jericho Road, Glenmont, Ohio, the employer had not provided adequate guarding on a machine used to convert commercial paper rolls into printed books, such as childrens coloring, activity and watercolor books and word search and crossword puzzle books.               1.  On the Goss Subway printer, serial number unreadable, there was no guard around any of the four turning, protruding external shafts near the unwind area on the right side of the machine near the employee walkway.  The shafts had approximately 1.5 inch diameter and protruded approximately 4 inches from the paper rolls.               2.  On the Goss Subway printer, serial number unreadable, there was no guard around any of the four turning, protruding external shafts near the unwind area on the left side of the machine nearest the wall.  The shafts had approximately 1.5 inch diameter and protruded approximately 4 inches from the paper rolls.

29 CFR 1910.141(d)(2)(i): Lavatories shall be made available in all places of employment. The requirements of this subdivision do not apply to mobile crews or to normally unattended work locations if employees working at these locations have transportation readily available to nearby washing facilities which meet the other requirements of this paragraph.  a.  At the work site, employees operated printing presses, stitchers and other machinery used in the production of paper activity books and handled chemicals used to clean the machinery.  The employer had not furnished employees with a handwashing facility that had tepid running water at the work site.

At the work site, employees had used all six portable fire extinguishers to extinguish a brush fire behind the building approximately one month prior to the inspection.  The employer had not ensured that fully charged portable fire extinguishers were available while the six used ones were being refilled.

Information and training may be designed to cover categories of hazards (e.g., flammability, carcinogenicity) or specific chemicals. Chemical-specific information must always be available through labels and material safety data sheets.  a.  At the work site, employees used various inks and a blanket wash cleaner to print paper books that included (but were not limted to) children's coloring, activity, watercolor books and others such as sudoku, word search and crossword puzzles. The employer had not trained employees on the hazards of chemicals used at the work site which included (but were not limited to) blanket wash.
