29 CFR 1910.179(n)(2)(ii): The load was not attached to the load block hook by means of slings or other approved devices: Slitting Dept.: 1) The lifting eye of the 20000 pound Cady Lifting Device used with the P&H Crane to lift and move coil pallets had a hairline fracture. 2) The design of the eye of the 15000 pound Cady Lifting Device, used to move coil pallets, is the type that failed four years ago and is causing wear on the hook of the crane.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: 1) Slitting Dept.: The paper insertion section of the slitter line was not guarded to prevent employee laceration injuries. 2) Adjacent to the Maintenance Dept.: The motor couplings of the five pumps transfering oil to oil buggies were not guarded. 3) Die Maintenance Shop: The chuck keys for two lathes were not the spring loaded type.
