29 CFR 1910.178(l)(1)(ii): The employer did not ensure that each operator had successfully completed the training required by paragraph (l), except as permitted by paragraph (l)(5), prior to permitting an employee to operate a power industrial truck:    a) 1100 Hummel Ave, Lemoyne Pa - Employees were permitted to operate a Hyster H90XMS forklift and a Hyster H110XL forklift and had not been properly trained, on or about June 5, 2013.    Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1910.178(p)(1): Powered industrial truck(s) found to be in need of repair, defective, or in any way unsafe had not been taken out of service until restored to safe operating condition(s):    a) 1100 Hummel Ave, Lemoyne Pa - Employees operated a Hyster H90XMS forklift which had been identified with needed repairs as noted by items such as, but not limited to: side shift hoses cracking; brake pedal hard - poor stop; needs front wiper blade; intermittent reverse - probable pedal control failure and were exposed to unsafe operations, on or about June 5, 2013.      b) 1100 Hummel Ave, Lemoyne Pa - Employees operated a Hyster H110XL forklift which had been identified with needed repairs as noted by items such as, but not limited to: no battery hold down; upper carriage slider bearings failed; replace air filter and were exposed to unsafe operations, on or about June 5, 2013.    No abatement certification or documentation required.
