29 CFR 1910.305(g)(2)(ii): Flexible cords were not used only in continuous lengths: At this location: Employees were exposed to electrical shock hazards from contact with an energized and severely damaged portable cable powering dryer shale shaker equipment. The damaged portable cable operated at 440 volts AC and was supported by a pipe welded to a nearby mud tank, at a height of 22 inches above the ground.

29 CFR 1910.334(a)(2)(ii): There was a defect or evidence of damage that could have exposed an employee to injury and the defective or damaged item was not removed from service: At this location: Employees engaged in workplace activities upon electrical equipment were exposed to shock hazards from contact with damaged flexible cords located 17 inches above wet ground and operating at 440 volts AC. Employees were engaged in diagnostic/repair activities of workplace secondary dryer shale shaker equipment that was powered by damaged flexible cords not appropriate for the conditions of use.
