29 CFR 1915.51(c)(3): When sufficient ventilation could not be obtained without blocking The means of access, employees in The Confined space were not protected by air line respirators:    a)  Wharf, Barge - Two workers arc welding in the compartment of a barge were not provided with airline respirators when the only sources of ventilation was a fan that was placed over and blocking the one means of egress, a 24 in. hatch, on or about 06/05/2013.   Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1915.74(c)(2): A safe walkway meeting the requirements of 29 CFR 1917.74(a)(7) was not provided to step safely to or from the wharf, float, barge, or river towboat:   a)  Wharf - Workers having to access the deck of a barge in the performance of their duties were exposed to a 3 foot fall for they had to climb 3 feet from the wharf to the deck without a ladder, gangway or other safe means of egress, on or about 06/05/2013.

29 CFR 1917.45(k)(4): A record of monthly inspections was not maintained for 6 months in or on the crane or derrick, or at the terminal:   a)  Marine Terminal Yard - The employer did not keep a written monthly inspection records for a Grove 28 ton rubber-tired crane (serial no. 71255R-TF28C) used for material and equipment handling on a marine terminal, on or about 06/05/2013.   Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
