29 CFR 1910.212(a)(5): Fan blade guard(s) were not provided where the periphery of the blades was less than seven feet from the floor or working level:    At the time of the inspection, two cooling fans were operated without guards. A floor fan was operated without a rear guard and a "Porta Cool" unit was operated with the front guard removed.    Abatement certification and abatement documentation is required for this violation.  The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos which you believe will be helpful.  The abatement certification sheet is enclosed with the citations.

29 CFR 1910.219(d)(1): Pulley(s) with part(s) seven feet or less from the floor or work platform were not guarded in accordance with the requirements specified in 29 CFR 1910.219(m) and (o):    At the time of the inspection the following pulleys were not guarded;    a) The drive pulley for the floor fan.  b) The drive pulleys for the air compressor.     Abatement certification and abatement documentation is required for this violation.  The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos which you believe will be helpful. 29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:    At the time of the inspection, lockout procedures were not utilized when employees were replacing a bolt in the ram of the Fox baler. The power supply to the controls on the ram were not locked to prevent the un-expected energization and startup of the hydraulic system including the ram.     The McCoy Salvage Co. Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.147(c)(4)(i), which was contained in OSHA inspection number 315249334 citation number 1, item number 6b and was affirmed as a final order on 4/2/2011, with respect to a workplace located at 1301 Adams Ave, Caruthersville, MO.

29 CFR 1910.305(b)(1)(ii): Unused openings in boxes, cabinets, or fittings were not effectively closed:    At the time of the inspection the following openings were found;    a) The circuit breaker panel box was missing multiple breakers at the northeast corner of the metal processing building.  b) The junction box above the shear had an opening that was not covered.  c) The disconnect box at the shear was damaged.  d) The motor for the Densor 600 was missing the cover.  e) The air compressor switch was not covered.      The McCoy Salvage Co., was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.305(b)(1)(ii) which was contained in OSHA inspection number 315249334, citation number 1, item number 9a and was affirmed as a final order on 4/2/2011, with respect to a workplace located at 1301 Adams Ave. Caruthersville, MO.
