29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652(b) or (c):    a.   On or about June 13, 2013, employees working in the 6 feet deep eastern trench were not protected from the hazards of collapse by the use of a protective system.    b.   On or about June 13, 2013, employees working in the 6 feet deep western trench were not protected from the hazards of collapse by the use of a protective system.

29 CFR 1926.701(b): All protruding reinforcing steel, onto and/or into which employees could fall or come against, was not guarded to eliminate the hazard of impalement:  a.   On or about June 13, 2013, employees working around the northeast corner of the job site were exposed to an impalement hazard from uncapped rebar rods extending 18 inches above the ground.
