29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:     On or about 28 March 2013 at the job site located at 2355 Lakeshore Lane, Cabot AR, the employer did not ensure employees used a hard hat while exposed to falling objects from employees engaged in construction activities above their heads.  This condition exposed employees on the ground to struck by hazards.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):       On or about 19 March 2013, at the job site located at 2355 Lakeshore Lane in Cabot Arkansas, the employer did not ensure employees exposed to fall hazards were protected while engaged in construction activities.  As a result the employees were exposed to falls of greater than 10 feet to the unfinished ground below.

29 CFR 1926.1053(b)(13): The top or top step of a stepladder was used as a step:      On or about 19 March 2013 at the job site located at 2355 Lakeshore Lane, Cabot AR, the employer did not ensure employees used a ladder as designed.  The employee used an 8 foot "A" frame step ladder to access the upper level of a new home constructions from inside the residence.  As a result employee were exposed to fall hazards greater than ten feet as they ascended and descended the upper working levels.
