29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):    a)  At 16 Ivy Loop Phenix City, Alabama:  On or about June 19, 2013 and at times prior; the employer allowed his employees to walk on top of the framed walls on the second floor of a residential home without any type of fall protection exposing them to fall hazards of 19.5 feet to the ground below.   b)  At 16 Ivy Loop Phenix City, Alabama:  On or about June 19, 2013 and at times prior;  the employer allowed his employees to walk on top of the framed walls on the first story of a residential home without any type of fall protection exposing them to fall hazards of 9.8 feet to the ground below.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  a)  At 16 Ivy Loop Phenix City, Alabama:  On or about June 19, 2013 and at times prior; the employer failed to train each employee on the hazards of falls to include but not limited to the nature of fall hazards, the correct procedures for erecting, maintaining, disassembling, and inspecting the fall protection used exposing his employees to fall hazards of up to 19.5 feet to the ground below.
