(a) 1224 Stark Ave. Columbus, GA: On or about  June 20, 2016, employees using pneumatic nail guns, and a circular saws, were exposed to eye injuries, in that; no eye protection was being worn.

29 CFR 1926.304(d): 29 CFR 1926.304(d): Each portable, power-driven circular saw withdrawn from  the work did not automatically and instantly return the lower guard to the  covering position of the saw.    (a) 1224 Stark Ave. Columbus, GA: On or about June 20, 2016:  Employee were exposed to lacerations while operating a Makita, Model 5007MG, and Rigid R3202, circular saws that had their lower guards defeated and could not return to their  covering position when withdrawn from the work.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:    a) 224 Stark Ave. Columbus, GA: On about June 20, 2016, the employer exposed an employee to a lower level fall hazard of approximately 12 - feet while working from a 6:12 pitched roof, in that; no means of fall protection was in use.    Exact Construction, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13) which was contained in OSHA inspection number 912994, citation number 1, item number 1 and was affirmed as a final order on 09/13/2013, with respect to a workplace located at 16 Ivy Loop, Phenix City, AL.
