29 CFR 1926.451(g)(1)(i): Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold was not protected by a personal fall arrest system. a) 1957 Fruitville Pike, Lancaster Pa - Employees were observed working from a ladder jack scaffold platform while not wearing a personal fall arrest system and were exposed to a 12 foot - 1 inch fall hazard, on or about June 28, 2013. No abatement certification or documentation required.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: a) 1957 Fruitville Pike, Lancaster Pa - Employees were observed using a 20 foot Werner portable extension ladder to access and egress a ladderjack scaffold and the ladder extended 11 inches above the landing surface exposing the employees to a fall hazard, on or about June 28, 2013. No abatement certification or documentation required.
