29 CFR 1926.1060(a)(1)(v): The employer did not ensure that each employee using ladders and/or stairways had been trained by a competent person in the standards contained in Subpart X:   a)  On or before 07-12-13, the employer did not provide employee ladder safety training for employees who did not associate fall from elevation hazards with utilizing a sixteen (16) ft. extension ladder to access/egress a roof surface with grabrails less than two feet above the upper landing and with descending the ladder without three points of contact.
