The fall distance was approximately 14 feet to the ground.

They told the csho that they had been verbally trained in fall protection, but also said they were not tied off on the roof because they didn't think they needed to be because it was low sloped.

29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed:    a) Jobsite, 220 Gentry Road, Moon Township, PA:  On or about July 17, 2013, 3 employees were working on a residential roof and using a Werner extension ladder for access that was extended only about 12 inches above the roof.
