29 CFR 1910.303(b)(1)(i): Electrical equipment was not free from recognized hazards likely to cause death or serious physical harm, in that the employer did not ensure that the electrical equipment had listing or labeling identifying the purpose for which the equipment was designed:  On the ex USS Vancouver, port side, 01 Amidships Level of the vessel: Employees engaged in workplace activities were exposed to electrical shock hazards from contact with energized portable cords not provided with approved strain relief devices as required by Article 400.10 (Pull at Joints and Terminals) of the National Electrical Code. The strain relief devices used for the worksite attachment of portable cables to panel board enclosures; were designed for use with non-metallic sheath cables (type NM) and not for use with flexible multiple conductor cables.

29 CFR 1910.305(a)(2)(viii): Suitable disconnecting switches or plug connectors were not installed to permit the disconnection of all ungrounded conductors of each temporary circuit. Multiwire branch circuits were not provided with a means to disconnect simultaneously all ungrounded conductors at the power outlet or panelboard where the branch circuit originated.   On the ex USS Vancouver, port side of Vessel, 01 Level, Amidships Area: Employees were exposed to shock hazards while engaged in workplace activities and attempting to disconnect a branch circuit from the panel board that consisted of 3 phase-separate plug connectors. The employer had installed 3 separate plug connectors for each branch circuit that operated at 120 volts AC at heights of 18 inches above the ground. The multiwire (3 phase) branch circuit was powered by 3 individual plug connectors colored red, white, black that were not physically connected to each other-these circuits were not provided with a means to disconnect simultaneously all ungrounded conductors at the power outlet. It was necessary for exposed employees to physically disconnect the three plug connectors; one plug connect at a time, so that the load was completely isolated from the power source. The employer used electric single pole welding plug connectors to power the respective multiwire branch circuits instead of using a means to disconnect simultaneously all ungrounded conductors at the power outlet.

29 CFR 1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screwsAt this location: Employees engaged in workplace activities were exposed to electrical shock hazards from contact with energized portable cords not provided with strain relief and connected to the following electrical devices:  On the ex USS Vancouver, at the following locations, adequate strain relief was not in-place:  a. Starboard side, weatherdeck, gangplank/forward area of vessel: The portable cord used to support employee workplace activities operated at 120 volts AC, with conductors attached to the terminals; but pulled loose from the male plug located 1 inch above the vessel deck surface,    b. Port side, 02 level, chart room area of the vessel: The portable cable with attached manufacturer installed light fixtures had pulled loose from the light fixture assembly. The light fixture assembly did not provide strain relief protection for the portable cable that was attached to the fabricated fixture assembly.

29 CFR 1915.82(b)(4): Lights and lighting stringers not designed by the manufacturer to be suspended by the electric cords were suspended solely by their electric cords:   At the ex USS Vancouver: Employees engaged in workplace activities were exposed to electrical shock hazards from contact with energized portable overhead lights not attached to compartment overhead areas, but suspended solely by the lighting stringer assembly. Portable lights in the following locations were suspended at a height of 70 inches above the deck and operated at 120 volts AC:  a. Port side of the Vessel, Forward end of the Well Deck,  b. Port side of the Vessel, 2nd Deck, Chart Room.

29 CFR 1915.132(a): The frames of portable electric tools and appliances were not grounded through a third wire in the conductors, or through a separate wire which was grounded at the source of the current.  Starboard Side, 300 Level, Forward area of the ex USS Vancouver: Employees were exposed to electrical shock hazard while engaged in shipboard activities using a cord and plug connected portable electric tool whose ground prong was missing from the male attachment plug. The energized reciprocating "sawzall" tool operated at 120 volts AC and was stored upon bulkhead pipe at a height of 48 inches above the deck.
