The service entrance was not insulated and employees were working from the aluminum platform that was not three feet away from the wiring.

29 CFR 1926.451(f)(7): Scaffolds were not erected, moved, dismantled, or altered, by trained and experienced employees under the supervision and direction of a competent person qualified in scaffold erection, moving, dismantling or alteration:; Such activities were not performed only by experienced and trained employees selected for such work by the competent person.    a)  16-18 Thompson Court, Enfield, CT. - On or about 7-19-13 - The ladder jack scaffold system being used by employees to install a new roof was not erected by or under the supervision of a competent person in that two platforms were bridged together.  There were only three ladders in use to support the two platforms and no access ladder was being used.  The center ladder of the scaffold system was a type II aluminum ladder and under rated for the intended load.

29 CFR 1926.454(a)(1): The employer failed to have each employee who performs work while on a scaffold trained regarding the nature of any electrical hazards, fall hazards and falling object hazards in the work area.    a)  16-18 Thompson Court, Enfield, CT. - On or about 7-19-13 - Employees were exposed to a 19 foot fall hazard while working on an improperly erected ladder jack scaffold system, platfoms bridged together on a type II aluminum ladder, aluminum platforms were within three feet of 240 volt electrical service entrance to the building.  Employees were not using fall protection while on the scaffold system.

29 CFR 1926.1053(b)(3): Ladders were loaded beyond their manufacturer's rated capacity:    a)  16-18 Thompson Court, Enfield, CT. - On or about 7-19-13 - The ladder used for a ladder jack scaffold system was loaded beyond the manufacturer's rated capacity.  The center ladder was used to support a bridged 44 foot long aluminum platforms loaded with 3 to 4 bundles of 85 lb. each of shingles and 4 employees.  The center ladder was a type II ladder rated at 225 lbs.
