The service entrance was not insulated and employees were working from the aluminum platform that was not three feet away from the wiring.

There were only three ladders in use to support the two platforms and no access ladder was being used. The center ladder of the scaffold system was a type II aluminum ladder and under rated for the intended load.

Employees were not using fall protection while on the scaffold system.

The center ladder was used to support a bridged 44 foot long aluminum platforms loaded with 3 to 4 bundles of 85 lb. each of shingles and 4 employees. The center ladder was a type II ladder rated at 225 lbs.
