29 CFR 1926.451(b)(1):  Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii)  (a) At Lot-14C of the Viera Builders development employees were working from the first and second level platforms of a tubular scaffold that were not fully planked exposing them to a 7-foot and 14-foot fall hazard.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface was not used:  (a)  The scaffold erected at the east wall of the two story residential construction site did not have ladder access exposing the four employees working from the scaffold to a fall hazard as observed on or about July 19, 2013.

29 CFR 1926.451(g)(1):  Each employee on a scaffold more than 10 feet above a lower level were not protected from falling to that lower level:  (a) At the two story residence under construction at Lot-14C a tubular supported scaffold was erected without a guardrail system or any other means of fall protection to protect the four employees from a 14-foot fall hazard while working from it.

29 CFR 1926.454(a):  The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards.  (a)  In the Cortona subdivision at Lot-14C where a supported tubular scaffold was being used, the employer did not provided training to the employees assigned with the erection of the scaffold and the prevention or elemination of fall hazards associated with scaffolding as observed on or about July 19, 2013.
