29 CFR 1926.501(b)(2)(i): Each employee who was constructing a leading edge 6 feet or more above lower levels was not protected from falling by guardrail systems, safety net systems, or personal fall arrest systems:      (a) Construction Site: On or about July 23, 2013, and at times prior thereto, employees were exposed to a fall of approximately eleven feet to the ground below while installing decking on the second level floor of a new residential home.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:    (a) Construction Site: On or about July 23, 2013, and at times prior thereto, employees were exposed to a fall of approximately eleven feet to the ground below due to lack of adequate training in the recognition of fall hazards and the means to control these hazards.
