29 CFR 1910.134(c)(1): In any workplace where respirators are necessary to protect the health of the employee or whenever respirators are required by the employer, the employer shall establish and implement a written respiratory protection program with worksite-specific procedures. The program shall be updated as necessary to reflect those changes in workplace conditions that affect respirator use. a. At the workplace, located at 3887 State Route 43, Richmond, Ohio, the employer did not establish or maintain a written repiratory protection program with worksite-specific procedures for employees required to wear full face, half face and airline respirators.

29 CFR 1910.178(q)(6): Industrial trucks shall not be altered so that the relative positions of the various parts are different from what they were when originally received from the manufacturer, nor shall they be altered either by the addition of extra parts not provided by the manufacturer or by the elimination of any parts, except as provided in paragraph (q)(12) of this section. Additional counterweighting of fork trucks shall not be done unless approved by the truck manufacturer. a. At the workplace located at 3887 State Route 43, Richmond, Ohio, a Yale forklift, model # GLC060VXNVSE085, serial # A910V07629D, was altered by the addition of 72 inch tine extensions without the manufacturers approval. b. At the workplace located at 3887 State Route 43, Ohio, a TCM forklift, model # FH615N8T, serial # A16S10131, was altered by the addition of 60 inch tine extensions without the manufactuers appoval.

29 CFR 1910.212(a)(3)(ii): The point of operation of machines whose operation exposes an employee to injury, shall be guarded. The guarding device shall be in conformity with any appropriate standards therefor, or, in the absence of applicable specific standards, shall be so designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle. a. At the workplace, a Magna Shear, Model # H-12014, Serial # 5961, did not have the shear blade guarded on the last three openings on the right of the hold-down mechanism exposing employees to an amputation hazard. b. At the workplace, in the router room, the small automated router table, did not have the router bit guarded, exposing employees to an amputation hazard. c. At the workplace, in the router room, the large automated router table, did not have the router bit guarded, exposing employeees to an amputation hazard.

In all cases, the working space shall permit at least a 90-degree opening of equipment doors or hinged panels; a. At the workplace, in the sanding room, an electrical panel was located in the corner with an air compressor permanently installed directly in front of it, not allowing the electrical panel door to be open 90 degrees.
