29 CFR 1926.251(e)(2): The rated capacity of the synthetic webbing sling was exceeded. On or about 07/20/2013 at 100 Graphic Packaging International Way, Macon, GA; an Ashley EE2-804 synthetic web sling used to support the lattice boom of a 200 ton Link-belt truck crane was loaded beyond its rated capacity, exposing employees to struck-by and fall hazards of 80 feet to the lower level.

29 CFR 1926.1403(b): The employer did not follow manufacturer procedures when an employer used synthetic slings during assembly or disassembly rigging: On or about 07/20/2013 at 100 Graphic Packaging International Way, Macon, GA; during the disassembly of the lattice boom of a Link-belt 200 ton truck crane, the employer used an Ashley EE2-804 synthetic web sling to support the lattice boom and used their own employer procedures instead of Link-belt's procedures, exposing employees to struck-by and fall hazards of 80 feet to the lower level.

29 CFR 1926.1404(j): Manufacturer limitations on the maximum amount of boom supported only by cantilevering were exceeded: On or about 07/20/2013 at 100 Graphic Packaging International Way, Macon, GA; an Ashley EE2-804 synthetic web sling was rigged to the lattice boom of a Link-belt 200 ton truck crane. The sling was located such that 113 feet of boom was cantilevered which exceeded the Link-belt limitation of 110 feet, exposing employees to struck-by and fall hazards of 80 feet to the lower level.
