29 CFR 1910.212(a)(1): Machine guarding was not provided to protect operator(s) and other employees from hazard(s) created by rotating parts:   a)  Facility, 15 Plum Street, Verona, PA:  On or about July 24, 2013, an employee was exposed to potential amputation hazards while operating the Bystar 4020 Laser cutting machine.  The equipment did not have a secured guard installed to protect employees from its rotating parts, such as, but not limited to the cutting head.

The equipment did not have a secured guard installed to protect employees from the laser cutting nozzle.
