29 CFR 1910.95(g)(6): At least annually after obtaining the baseline audiogram, the employer did not obtain a new audiogram for each employee exposed at or above an 8-hour time-weighted average of 85 decibels: a) In the facility, on or about July 26, 2018 - The employer did not obtain a new audiogram annually for each employee exposed at or above the 8-hour time-weighted average of 85 decibels. Employees were last provided an audiometric test in March 2015. Employees were exposed to noise levels in excess of the action level 85 dBa. The Aloma Shim and Manufacturing was previously cited for a violation of this Occupational Safety and Health Standard 29 CFR 1910.95(g)(6) which was contained in OSHA inspection number 1040827, citation number 1 item number 1b and was affirmed as a final order on March 26, 2015, with respect to a workplace located at 15 Plum Street, Verona, PA 15147.

Employees used solvents such as but not limited to: Renoform 057806 which contained triethanolamine, Denatured Alcohol which contained 95% ethanol, Electrolyte which contained magnesium chloride and citric acid, and cutting oil which contained hydrotreated heavy paraffinic distillate during metal shim manufacturing.
