29 CFR 1910.178(o)(2): Load(s) were being handled which exceeded the rated capacity of the industrial truck(s):     a)  On or about July 22, 2013, employees was exposed to a struck-by hazard while operating a Caterpillar forklift, model DP70E, sn:  AT20C70275, whose forks were replace with a John Harder/Spec, SF-72 Attachment.  The forklift was de-rated to a lifting capacity of 3850 lbs.
