Tail rods or extension piston rods were not guarded in accordance with paragraphs (m) and (o) of this section, or were not guarded by a guardrail on sides and end, with a clearance of not less than fifteen (15) nor more than twenty (20) inches when rod is fully extended:    On or about 7/24/13, and at time prior there to,  mud pumps area of well site  Justin 1 H  3, adequate guarding was not in place to protect employees from caught-in hazards of  the reciprocating piston/plunger parts of the operating mud pumps.
