29 CFR 1926.651(k)(2): Where the competent person found evidence of a situation that could result in possible cave-in, indications of failure of protective systems, hazardous atmospheres, or other hazardous conditions, exposed employee(s) was/were not removed from the hazardous area until the necessary precautions have been taken to ensure their safety:  (a) On or about July 24, 2013, for three employees exposed to cave-in hazards while installing sewer pipe from the nine-foot high vertical side walls without protective systems in place on the northeast corner of the excavation located at the intersection of 16th Street NE and Fourth Avenue NE, Watford City, ND.
