29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:    On August 6, 2013 at the jobsite located at or near 200 Jeffreys Street in Pleasant Hill, MO employees were exposed to a fall hazard in that they were working on a steep roof over 6 feet above lower level without an approved form of fall protection.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladders length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:    On August 6, 2013 at the jobsite at or near 200 Jeffreys Street in Pleasant Hill, MO employees were exposed to a fall hazard in that the ladder they were using to access and egress the roof did not extend at least 3 feet above the upper landing.
