Worksite located at 416 SE 3rd Street, Lee's Summit, Missouri- Employees were observed working and walkiing directly beneath elevated employees and materials which could hit employees below who were not wearing hard hats.

Worksite located at 416 SE 3rd Street, Lee's Summit, Missouri- Employees were performing work such as, but not limited to, cutting lumber with a chain saw and were not provided with any means of eye and face protection in case of splintering or kick-back hazards.

Worksite located at 416 SE 3rd Street, Lee's Summit, Missouri- Employees performing demolition and the rebuilding of the exterior decks were not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.
